How to enroll in CareASSIST

What's next once you've submitted your enrollment form?
Your Case Manager will:
Reach out to you and your doctor within 1 to 2 business days to introduce themselves, get to know you, and understand your needs.
Complete a benefits investigation to assess your insurance coverage, discuss your options for financial
assistance to see if you qualify, and share additional support resources.
Answer any questions you might have and continue to check in, as needed, to provide personalized support throughout your treatment.
