What's next once you've submitted your enrollment form?
A dedicated Case Manager will reach out within 1 to 2 business days. They'll introduce themselves to you and your patient. They'll also review your patient's specific needs.
We will conduct a benefits investigation to assess your patient's insurance coverage, eligibility for financial assistance, and if they qualify for additional support resources.
Your CareASSIST Case Manager will continue to partner with you throughout your patient's treatment.
